This is the complete report online for you to put at your site. Just set it up as is; you can put your own name on the order form. Go to your file menu, select "Save As" an html (sometimes called a Source) file. You can also put this on a floppy disk to distribute at conventions, seminars, meetings, whatever.

Disks cost about ten cents a piece; much easier than distributing paper. There is a pdf file below of the same report as well.
